Got Arizona’s DDD? Let’s Get Started!
If your child qualifies for therapy through Arizona’s Division of Developmental Disabilities (DDD), Therapy Accomplished is ready to team up with you!
Just give the crew a call at (928) 377-9867 to check scheduling compatibility and let them know you’re with DDD. Here’s the game plan:
- Grab a prescription from your doctor for “PT Evaluation and Treatment.”
- Notify your DDD support coordinator that your child will be receiving therapy with Therapy Accomplished to authorize the evaluation.
- After the evaluation, Therapy Accomplished sends the completed evaluation and plan of care to your doctor for a signature.
- The signed plan goes to your support coordinator to authorize ongoing therapy. With these steps, your child will be zooming toward their goals with Therapy Accomplished’s fun, family-focused approach in no time!
